These rates reflect a 10% discount off the standard rate for payment at the time of service.
We are in‐network with Kaiser, Moda, and Pacific Source.
You may be eligible for out‐of‐network benefits. We can provide you with a Superbill (receipt) that you can use to process your reimbursements directly.
If you need to cancel your appointment, please do so within 48 hours of our appointment to avoid a late cancellation fee of $100.
No‐show appointments will be charged at the full fee of $180.
